Hi Xinjie,
I also faced this problem last week when I was simulating a voxelized phantom using the voxelized source for a dosimetry simulation. But I found that this is due to the source definition; I was using the beta spectrum simulation, but when I change the source file format using the histogram and discrete spectrum type, it was gone.

And it looks like the root output data were very small whenever I got this warning, I have no idea whether there is any correlation between the warning and the small output, and I am curious whether this warning is a severe problem or neglectable.
