Dear Andinet,
Take a look at "examples/example_PHANTOM_SOURCE/Voxelized_Phantom_Source"
in Gate source directory.
One more, in fact this is the best, GATE User's guide is pretty good.

ps. It's amazing that kitware needs to PET images. :)
Are you gonna develop some image processing for PET in ITK?
